HSS 201 - Case Management Credits: 5 Students will explore case management in a wide variety of human services organizations and learn skills for developing, implementing, and monitoring effective case plans that help clients achieve self-sufficiency. Course Outcomes:
- Explain how the different realms of human services govern the role of case workers and managers and stages of case management.
- Demonstrate the principles and process of case management to streamline flow of paperwork and information to and from clients and workers to help facilitate the processing of cases and applications.
- Apply case management skills, including documentation, assessment, treatment, planning, crisis intervention, and appropriate referrals to meet client and agency needs.
Course Topics:
- The case management cycle
- Helping clients define goals for employment and services
- Developing and modifying the case plan
- Fostering client responsibility for overcoming barriers and reducing resistance
- The case manager’s role
