AHSE 144 - The Medical Team Credits: 1 This course defines the role, skills, and function of patient care navigation and how it fits into the care team. Also, it explains the need for effective patient navigators in today’s complex healthcare system. This is the third course of a seven-course series. Course Outcomes:
- Describe the significance of an interdisciplinary team and the patient navigator’s function on the team.
- Identify professional scope of practice of a patient care navigator.
- Explain how to locate appropriate patient community resources and establish eligibility to access resources.
- Explain the relationship between care coordination, plan of care and continuum of care.
- Identify barriers to care coordination.
