AHSE 143 - The Patient Experience

This course focuses on the whole patient, including cultural considerations, barriers to optimal health care, and the impact of chronic illness, and discusses promoting individual wellness using defined advocacy and engagement techniques. This is the second course of a seven-course series.
Course Outcomes:
  1. Apply the whole person theoretical framework in identifying patient needs.
  2. Discuss why chronic disease management is affected by knowledge of whole person factors.
  3. Apply well-defined advocacy and engagement techniques to promote individual wellness.

Course Topics:
  1. The whole person concept of wellness
  2. Principles of wellness
  3. Impact of chronic disease on the patient, caregivers, and families
  4. Chronic diseases: CHF, MS, Diabetes, COPD, Asthma, A-Fib, and Depression
  5. Role of care coordination in relation to chronic disease
  6. End of life issues and palliative care
  7. Patient activation, advocacy, and engagement
  8. Patient navigator role in empowerment and engagement
